But I can't help but notice how two prominent industries rarely offer discounts or incentives to attract customers.

These industries where companies typically do not offer discounts or promotions to attract customers often fall into categories where demand is consistently high, and the products or services are considered essential or specialized. Pharmaceuticals and healthcare is one notable industry, and the second is utilities.

Utilities comprise one of the most defensive segments of the economy because households and corporations keep the lights on even when the economy is in its darkest hour. Yet, this sector pulled back in 2023 as investors shifted their favor away from defensive stocks towards CDs and money-market funds.

A decline in interest rates would be a positive for utility stocks, as it would make the dividends they pay more attractive (and competitive) in comparison, and investors will see significant capital upside. Regulated utilities have the ability to pass through incremental debt financing costs and rising prices to customers through rate-base requests with regulators. The inelasticity of demand and strong pricing power make utilities excellent to own amidst an economic slowdown.

During the COVID-19 pandemic, the healthcare industry saw billions of dollars of inflows in heavy anticipation for vaccines and treatments. In this post-pandemic world, innovation is high in other areas, and this industry is entering the new year with compressed valuations.

Several areas, like medical devices, have experienced steep sell-offs due to uncertainty and doubt about the impact on their business from new anti-obesity drugs whose stocks have achieved high double-digit returns. These drugs work by mimicking gut hormones that regulate appetite and have achieved unprecedented levels (15-20%) of weight loss in patients.

Novo Nordisk's ( NVO ) Wegovy demonstrated a reduced risk of heart attack, stroke, and death in people with cardiovascular disease and obesity by 20%. This class of drugs, termed GLP-1, have been marketed for more than a decade for diabetes, and new benefits are still being uncovered, including in cardiovascular, liver, and kidney diseases. As such, these drugs are in severe shortage, making countries ban the prescription for non-diabetic purposes.

The broader range of applications of these drugs have raised concerns about the future of medical device products, surgical equipment, and drugs that treat related diseases like sleep apnea, heart disease, orthopedics, etc. After all, if there is no obesity, there won't be the need for any surgeries - or at least that is what Mr. Market believes.

However, industry experts believe surgery and treatments will have relevance adjacent to these GLP1s and that these drugs can make more patients eligible for them. Moreover, these are expensive drugs, often not covered by insurance, whose benefits last only until the patient is on the course. As such, the market is wrong in writing off the rest of the healthcare sector in favor of a few drugs. We like the valuation of this sector and would like the dual benefits of tailwinds from soaring demand and short supply of these GLP-1 drugs and the cheap valuation across other sub-sectors.

THW 's top 10 holdings comprise 40% of the fund, with significant allocations to Eli Lilly ( LLY ) and NVO. THW currently trades near par with NAV, making it a rare opportunity to buy into the sector's current discounted pricing. The CEF's $0.1167/month reflects a sizable 11.3% annualized yield. Notably, the fund's two monthly distributions in FY 2024 were 100% Return of Capital, while for FY 2023, were 85% ROC, 1% NII, and 14% LT Gains (Fiscal Year ends Sept. 30th).

THW operates with a 20% leverage, with a borrowing rate of SOFR plus 0.75%. With SOFR currently at 5.3% , the fund faces negative pressure on the Net Investment Income due to higher interest expenses. THW will be a significant beneficiary from upcoming rate cuts, as it would immediately lead to bottom-line improvement and book value improvement.

Global fertility rates are dropping, and developed nations are graying fast. The need for innovative healthcare solutions is more pressing than ever in the domains of prevention and cure. Ozempic may have a bright future in making the world population healthier, but medicine is rarely binary. Most conditions are best managed with multiple therapeutic classes and rarely completely cured.
